The Surgical Treatment of Proximal Femoral Nail for Peritrochanteric Fracture of Femur

Abstract

|
|
Purpose: A retrospective study to evaluate the surgical treatment of a proximal femoral nail in a pentrochanteric fracture of the femur.
Materials and Methods: After retrospectively reviewing the medical records of 32 patients diagnosed with a peritrochanteric fracture of the femur, and who were managed with a proximal femoral nail in our department, between March 2000 and June 2002, the fracture characteristics were classified using the method of Evans, and the patterns of trabecular loss assessed using the classification of the internal trabecular system proposed by Singh et al. The patients¢¥ maintenance of fracture reduction, degree of bone healing and the presence of complications were evaluated on the basis of the radiographic and clinical findings at follow-up.
Results: In our patients, 24 cases (75%) had unstable fractures, and in terms of the degree of trabecular loss, 19 (61.9%) had a Singh index below 3. The patients¢¥ mean surgical time and bleeding amount were 61 minutes and 250 cc, respectively. Mean time to postoperative partial weight bearing was 1 week. One postoperative infection was noted in our group, but no lag screw cutting out was noted during the follow-up.
Conclusions: A proximal femoral nail will be prevalence than that of a gamma nail due to its biomechanic merit. However, it may be of great advantage in the transition of delayed screws, or the perforation rate of the femoral head, to use metal nails that require further follow-up.
